Our home health agency is committed to providing quality care for our clients in their own home environment and our employees are dedicated to empowering our customers to lead richer, fuller lives and have consistently recognized Ecumen as a “Best Place to Work,” an award by the Minneapolis-St. Paul Business Journal.Job DescriptionThe home health Registered Nurse is responsible for providing direct home health nursing care for a team of clients in accordance withnursing practice, Medicare regulations, agency standards and policies.Specific responsibilities include:Completes and documents skilled nursing assessments for patients following physician orders.Collaborates with physicians, other health care professionals, clients and families in developing a comprehensive coordinated plan of care.Develops an individualized plan of care which includes objectives goals for client improvement in health and functional status. Evaluates plan of care on a continuous basis, updates physician(s) and other therapies of changes in client status and need for revision of plan of care.Delivers skilled nursing services efficiently and effectively to meet client care needs.Ensures that physician orders are communicated to the agency's staff with accuracy, thoroughness and understanding.Initiates case conferences to discuss multidisciplinary team responsibilities, client progress, plans for continued care, new goals, etc.Plans for discharge from the agency with the client and caregivers. Makes referrals for alternative level of care if necessary.Follows schedule frequency as ordered by physician.Proactively participates in care conferences with other health team members as needed.Maintains privacy and confidentiality of client, client's records and client's living environment. Understands and follows HIPAA guidelines.Promotes personal safety and a safe environment for clients and co-workers.Other duties as assigned.QualificationsThe following are the minimum required qualifications for the home health RN position:Graduate from an accredited school of nursing as a Registered Nurse (RN).Maintains a current, unencumbered Registered Nurse license in the State of Minnesota.One (1) year experience as a Registered Nurse in home health.Knowledge of Medicare home health regulations and OASIS data-sets.Demonstrated leadership and judgement with strong knowledge in nursing practices.Must understand and communicate with specialized medical terminology and explain procedures to clients.Valid driver's license, current auto insurance and reliable vehicle.Ability to communicate effectively, including written and verbal skills.Ability to work work well both individually and as part of a team, through respect, collaboration and dedication; successfully interact with a diverse culture base.Ability to prioritize and organize work effectively and efficiently.Demonstrated positive attitude, high integrity and ethical awareness.Proven to be dependable and responsible.Compassionate and caring attitude.Additional InformationAPPLY ONLINE: http://www.ecumen.org/careersEOE/AA; Drug Free Workplace
